We’re honored you’ve chosen us to care for your keiki. Here’s how to get started as a new patient:

What to Expect as a New Patient:

  • Arrive 10–15 minutes early for check-in
  • Fill out New Patient Intake Forms
  • Bring a valid ID and insurance card to your first visit
  • Transfer previous medical records, if available
  • Prepare a list of any current medications or allergies
